1. If it is your first ever medical (i.e. an initial) you need to register on the UK CAA customer portal, obtain a UK CAA reference number and apply for medical services as a pilot to use CELLMA. 3. If you have a complex medical history or are on longterm medication or have been given new diagnoses or treatments since the last medical, please consider emailing us with the details beforehand as we might require GP records, specialist reports or discharge letters to issue the medical.

4. For an initial medical, obtain a printout of your latest opticians report (ideally within the last 2-3 years). If you had significant eye problems in the past, or wear high prescription spectacles please print this form, and take it to an opticians to fill out. If you have never been to an opticians, please make an appointment and bring their report with you. Please bring your spectacles or contact lenses (if applicable) with you to the medical.

5. Fill out the UK CAA mental health assessment form, print, and bring with you to the medical. Do not date or sign it.

6. Please inform us as soon as possible if you would like a chaperone, unless you booked through our online system (which prompts for chaperone request).
